Required

Requirements: [UT Board Reg. R156-17b-302]

1. Applicant must pass the Board’s criminal background check.

2. Applicant must complete a training program approved by the board.

3. Applicant must submit an application ($95) to the pharmacy board.

4. Applicant document current certification by a national, board-approved pharmacy technician certification exam.

5. Applicant must submit three blueprint finger cards. (Form FD-258)

Renewal
Required

Applicant must submit a renewal application ($47) to the pharmacy board.

Training
Required

Individuals must complete a board-approved technician training program. The training program must require at least 180 hours of practical training supervised by a licensed pharmacist. The pharmacy can adopt The National Pharmacy Technician Training Program as part of its training program that is submitted to the Board for approval. Training also includes 180 hours of practical experience. [UT Board Reg. R156-17b-304]

Ratio
N/A

The technician-to-pharmacist ratio is 3:1 and only one of the three technicians can be unlicensed.

Continuing Education
Required

Applicant must complete 20 hours of qualifying continuing education. At least eight hours must be live lecture or seminars and one hour must cover pharmacy law and ethics. [UT Board Reg. R156-17b-309]

Can the ExCPT be Taken?
Yes

Certification is required by a board-approved program. The ExCPT is approved by the Utah Board of Pharmacy.
